đ§ Brain Longevity Part 2 â
 Is your Brain running hot?

Last month I began my Brain Longevity Series, starting with the blood markers we can investigate long before we experience cognitive symptoms. I also introduced Dr Dale Bredesen's different patterns of cognitive decline: the inflammatory "Hot" brain, the atrophic "Cold" brain, the glycotoxic "Sweet" brain and the "Toxic" brain.
This month, let's turn the temperature upâor rather, learn how to turn it downâand look more closely at the Hot Brain.

đĽ What is a "Hot Brain"?
Inflammation isn't inherently bad. We need it to fight infection, respond to injury and initiate repair. The problem begins when an inflammatory response that should be temporary becomes persistent.
In Dr Bredesen's framework, Type 1 or inflammatory Alzheimer's disease describes a pattern in which ongoing inflammation is one of the contributors to cognitive decline. And this is where I think Alzheimer's prevention becomes particularly interesting: the source of that inflammation doesn't necessarily begin in the brain.
It may involve metabolic dysfunction, chronic infection, poor oral or gut health, disrupted sleep, environmental exposures, excess visceral fat, stress or inflammatory dietary patterns. This is why I don't like looking at the brain as an isolated organ inside the skull. The brain is listening to what is happening throughout the body.
Last month I mentioned hs-CRP, a useful blood marker of systemic inflammation. But if it is elevated, the interesting question isn't simply "How do we lower CRP?" It is "Why is this person's immune system producing an inflammatory signal in the first place?"
That is where the detective work begins.

đĽ Food is One Part of the Conversation
Food certainly isn't the only cause of chronic inflammation, but it is one of the environmental signals we encounter several times every day. It affects our microbiome, glucose regulation, immune system and metabolism while providing the raw materials for our hormones, neurotransmitters and cell membranes.
And here's the important part:Â there isn't one perfect anti-inflammatory diet for everybody.
A food that works beautifully for one person may not work quite so well for another. Sometimes we don't discover this until we temporarily remove certain foods and then systematically reintroduce them.
This is precisely why I like a properly structured elimination diet.

đż And Perfect Timing: We're Doing It Together in September
In September, I'll be running another Anti-Inflammatory Elimination Diet group programme, and this month's Hot Brain discussion felt like the perfect introduction.
This is not a calorie-restricted diet and it isn't a weight-loss programme. Instead, we'll temporarily remove common potentially reactive foods while building meals around nourishing whole foods, good-quality protein, fibre, colourful plants and healthy fats.
Then we reintroduce.
That's actually my favourite part, because instead of following somebody else's list of "good" and "bad" foods, we start gathering information about your own biology. We can pay attention to digestion, skin, joints, energy, sleep, mood, concentration and other symptoms as foods return.
Think of it as a controlled conversation with your body rather than another diet.
And because we're talking about the Hot Brain this month, I'd particularly like participants to notice something we don't always associate with food:Â their cognitive function. What happens to your clarity, concentration, afternoon energy, sleep and mood during the programme?
The purpose isn't to stay on an increasingly restrictive diet forever. Elimination is the experiment; reintroduction is where we learn.

đ§ One Thing to Take Away This Month
If I leave you with one thought, it's this:Â don't automatically try to suppress an inflammatory signal without asking why it is there.
Inflammation can be information.
Perhaps it is coming from metabolic health, perhaps the gut or mouth, perhaps sleep, an infection, stress, environmental exposureâor perhaps food is contributing to the picture.
Our job is to investigate rather than assume.
That's exactly what I want this Brain Longevity Series to be about: understanding the biology beneath cognitive decline early enough that we have an opportunity to support it.

Next month we'll continue the journey and explore another fascinating pattern: the "Sweet Brain", where glucose, insulin resistance and brain health meet. And yes, this one may change the way you look at your blood sugar!
